If you stop paying
Nothing is deleted. Older history shows as one locked row until you come back, extra medicines are paused rather than removed, and backup and export stay free so your data is always yours.
Subscriptions are billed by Apple and renew until you cancel in your Apple Account settings. Prices are US dollars and may vary by country.
